The publisher, Hermann Wunsche, was one of Warhol's closest friends and he suggested that Warhol look at iconic European images in the same way Warhol had previously looked at American Icons in the Myths series and his works featuring Marilyn Monroe.
According to the catalogue raisonne, this work is based on Karl Josef Stieler's 1819 painting "Beethoven." The musical notes in the image are from Beethoven's 1801 "Moonlight" sonata, Opus 27, no. 2.
